Treatment Agreement
- I understand that results may vary from person to person and that an exact result cannot be predicted.
- I understand that completing a full treatment series for which schedule is to be administered by the consultant is necessary to maximize treatment efficacy.
- I agree to before and after treatment photographs and measurements as this will help in the evaluation of the results of the treatment.
- I agree to a treatment plan with a validity period of one (1) year. In the event I am pregnant, I may be granted an extension of my treatment plan up to a period of six (6) months.
- Late Cancellation Policy applies for all clients. A penalty of 3 MCs will be deducted from the plan when there is a no-show or cancellation made less than 24 hours before the appointment date for the third time.
