It is derived from lithotripsy, a technology that has been in use for approximately 20 years to disintegrate kidney stones.

Chronic musculoskeletal conditions such as tendinitis, are often associated with a substantial degree of scarring and calcium deposition. Calcium deposits may restrict motion and encroach on other structures such as nerves and blood vessels, thus causing pain and impairment of function.
EWST disrupts these calcific deposits with shock waves, thereby loosening adjacent structures and promoting reabsorption of calcium. Pain is decreased and function improved - allowing restoration of normal range of movement.
Microtrauma induced by ESWT may promote angiogenesis (formation of new blood vessels) and thus aid in healing.
Finally, shock waves have been shown to stimulate osteogenesis (formation of bone) and promote callous formation, which is the rationale for ESWT in delayed union or non-union of bone fractures
